Why Dr. Kish?

Shoulder Pain

I have treated and helped numerous patients with shoulder pain. Many have been through long term PT and have been on pain medications for weeks, months or years. Many also have been told their problem does not require surgery, or that they will have to live with it. Even in chronic cases, there is a lot that can still be accomplished to return them to a pain free and functional state. So what causes shoulder pain?

  • Some cases involve incomplete tears of one or more rotator muscles. If treated and rehabilitated correctly, these will heal and become reasonably functional. Other cases involve ligament or bone injuries, and depending on their type and severity, you may still be able to rehab your injury.
     
  • In most cases there is a major muscle problem which has been undiagnosed and therefore left untreated or under-treated. This muscle problem is in itself the cause of most shoulder pain. This muscle problem is called Myofascial Pain and Dysfunction, or MFPD. MFPD is a long term muscle spasm that has caused damage to the areas of the muscle or the entire muscle. As a muscle spasms it becomes overused requiring increased oxygen and nutrition. Yet, the most compressible part of a muscle is it's own blood supply. Therefore as the muscle requires more blood flow to stay healthy, it's own spasm diminishes it's ability to get it, and the damage is extended.
     
  • From the prolonged spasm, nodules of scar tissue and pain nerve fibers form Trigger Points, or TP. These muscle knots may be the size of your thumb pad or as large as the muscle itself. Typically a person will develop postural and movement patterns to minimize the pain and further engrain the problem.
